CNA Reactivation Applications

If you hold a CNA certificate that has been expired for 61 days or more, you have two options to reactivate your certificate (choose one):

1. You are a CNA who either completed a training program within the last two years, or you can meet the work requirement of having at least 400 paid work hours (doing CNA authorized duties under the supervision or monitoring of a RN) in the past two years, and your certificate must have been expired, retired, or inactive prior to 12/31/2019.  Applicants whose certificate was active at any time after 1/1/2020 are not eligible.  
   
   
2. You are a CNA who is one of the following:

• Meets the work requirement of having at least 400 paid work hours (doing CNA authorized duties under the supervision or monitoring of a RN) in the past two years, but your certification expired on or after 1/1/2020.
• a student nurse, 
• a nurse who has graduated within the last year, or 
• a licensed LPN/RN that is wanting to reactivate your expired CNA certification. 

You are not eligible for fee waivers.
